Your GP practice has a Social Prescribing Service
Social Prescribing can help you with non-medical issues that may be negatively affecting you, for example stress, money worries, housing problems, loneliness, unemployment. Or it can help if you are looking for ways to improve your lifestyle and feel happier; get more involved in leisure and community activities or volunteer work.
Ask your GP, healthcare professional or one of the GP reception team to refer you to the Social Prescribing Service available within your GP practice.
Social Prescribing is available to anyone over the age of 16 and can help you with non-medical issues that may be affecting your overall health and wellbeing.
After you’ve been referred to the Social Prescribing Service, you will be contacted by a Social Prescribing Link Worker and offered a 1:1 appointment in person or over the phone to discuss what’s important to you.
Your link worker can help you to consider local support options, services or activities that may help you to improve your overall wellbeing, your ‘Prescription for Wellbeing’.
You may be offered one or more follow up sessions to review your progress and see if your situation has improved via the support you’re being linked to.
Ask at your GP practice today for a referral to the Social Prescribing Service which is part of the wider support available to you via Haydock and Newton Primary Care Network within your GP practice.
